Local causes are often the obvious source of penile pain, but referred pain from elsewhere in the body is common too. Bladder stones often present as pain at the tip of the penis. Some urinary tract infections and prostatitis can also cause penile pain.
While treatment focuses on the underlying cause, some patients experience nerve-type pain, known as neuropathic pain. This is nerve hypersensitivity in the penis, often caused by irritation or nerve injury from another condition. Even after the original medical issue is treated, a nerve that has been sensitised can continue to cause chronic penile pain. Because the penis has a dense network of nerves, this nerve-type pain can be difficult to reverse once it has set in.
